[KPFARS-Operations] 2017 Squad ID Cards

We have started to issue the 2017 Squad ID Cards.  To date, only 15 people
have completed the form for a card.  

I need to complete this project by the end of this month.

 

If you want a 2017 KPFARS ID, here are your options:

 

Option 1.

Please follow the instructions below.

 

1.	All fields in the following form with an "*" must be completed.
2.	Other fields are optional.
3.	The "Photo" field allows you to attach a photo to the form being
submitted.
4.	Photos should be a Passport Type photo taken in front of a plain
Light colored wall. (Forward looking head shot, NOT Full Body) The hallway
wall between the restrooms at the building on New Road is a good example of
the type wall to use.
5.	Submit the form when complete.
6.	Click HERE

Option 2.

Email a picture to me as described in #4 above.  Include the Correct
spelling of your First and Last name for the card, and if you are an EMT.

Have someone take your picture and send it to me. See Option 2.
