Subject: IMPOTANT: State Waivers for ALS and BLS Providers Triaging Possible Covid-19 Patients to Home.
While technically these waivers apply only to licensed BLS providers, it has always been the policy of our squad to transport patients to an appropriate health care facility of their choice upon their request, whether we think they need to go to the hospital or not. However, with the large number of patients showing mild to moderate Covid-19 symptoms, the State is recommending that we triage patients and only transport those who will benefit from hospitalization.
Basically you should follow the triage protocol attached to the waiver. We will have a copy in each ambulance. If the protocol indicates that a patient should be transported then encourage them to go to the hospital and obtain an RMA is they refuse. On the other hand if the patient's triage indicates they should be triaged to home, contact the hospital he wants to go to and have them determine whether the patient to be triaged to home. If the physician concurs that the patient should be triage to home advise the patient and let him talk to the physician. Make sure you document the physician/advanced practitioner you spoke to. you spoke to. Calling NJPIES (1-800-222-1222) is not currently an option as wait times are about 1 hour to speak to a health care provider. If you cannot reach online medical direction, you should advise the patient based on the triage protocol, but do not refuse a patient without medical direction. When triaging a patient to home, leave them a copy of the follow-up instructions, which we will place in the ambulance.
