[KPFARS-Operations] Blue Light Permit Application

Attached is a blue light permit application.

If you want a permit to use a blue light, please fill out the attached application.

Depending on your .pdf software, you may be able to download and/or complete the form on your computer by filling in the appropriate highlighted blue fields.  Otherwise, you can print the form and complete by hand.

Fill in your name, your address, including street address, city, county, and zip code.
Enter your N.J. driver's license number.
Check "volunteer" if not pre-marked.
Then check either "initial" (if this is your first application), "renewal" (if you have a blue light permit but it is expiring or has expired), or "duplicate" (if you are seeking to replace a current permit that is lost, damaged, or destroyed).
Finally, sign the application where it says "volunteer's signature.

When done, place the application in the chief's mail slot by Monday, February 24th.  I will then complete the form and drop them off at the township offices.

The squad does not supply blue lights, but the application describes the restrictions on the blue light and it can be purchased at any number of stores, including online at Amazon, etc.
