Good Evening All

PLEASE READ THIS ENTIRE EMAIL AND SHARE WITH YOUR SQUADS

I was contacted today by the DOH with an update to the process for EMT training fund  reimbursement..(The ultimate goal is still for this to be an entirely electronic platform but they must take several small steps to get there)

Effective immediately when applying to use the training fund for any courses which start on or after August 1, 2025 you must use the attached form (EMS62). Please follow the directions below.

For any classes prior to August 1, please continue to use the email to the DOH.  If you have completed the email for a class that starts after August 1, please complete the attached form.

For classes starting on or after August 1, 2025
1. Have the student complete the attached form (EMS62)and sign it electronically (no wet signatures will be accepted,)

2. If the student is under 18 years of age a parent or guardian must electronically sign the form.

3. The student should then forward the form to one of the squad officers authorized to sign the training fund form.  That officer will sign the form electronically and return it to the student.

4. The student will then email the completed/signed form to the training agency or instructor at the email designated for this purpose by the training agency/instructor.

There is no need to send the form to the DOH - it should only be sent to the training agency/instructor.

All signatures must be ELECTRONIC, no wet signatures will be accepted.

Also please note that the NJOEMS website will be down on Monday and Tuesday (July 7 & 8) for maintenance.

I know the form says Head/Chief Officer but DOH has assured me that space is for any of the three designated squad people to sign off in that space.
